Service 1 · Medicare Advantage Options Education

Medicare Advantage plans often end coverage too early

Plans tend to close out goals quickly, sometimes while a resident is still making real progress in therapy. The cost of that cut shows up later.

Cut short

Coverage ends while the resident is still progressing.

Rushed home

No time to arrange aides, equipment and support at home.

Readmitted

An avoidable return to the hospital.

We make sure residents understand their options, so the decision about their care is theirs.

The resident journey

Same resident. Two very different outcomes.

When coverage is cut short

  1. Admitted for rehabMaking steady gains in therapy.
  2. Plan ends coverageOften around three weeks, mid-progress.
  3. Rushed homeNo time for aides, equipment or support.
  4. Back in the hospitalAn avoidable readmission.

With Patient Priority

  1. Admitted for rehabScreened with nursing and rehab.
  2. Educated on optionsBalanced information. The resident decides.
  3. Finishes recoveryReaches their best level of function.
  4. Home, and stays homeA safe, planned, lasting discharge.

Why outside, not in-house

We own the process, start to finish

Your team is already stretched. We bring a dedicated consultant into your building who handles the education and the follow-through.

Handled in-house

  • A side job for busy staff
  • Not insurance specialists
  • Can feel like a revenue push to families
  • Easy to miss a step

With Patient Priority

  • One consultant, on site monthly or as needed
  • Every plan screened before any conversation
  • Balanced, resident-first education
  • Every step documented, with informed consent

We educate. Residents decide. Your team processes the form.

The Patient Priority principle

How it works

A clear, documented process every month

1

Screen with your team

We review residents with your DON, nursing and rehab to find those still making progress.

2

Educate the resident

We explain every option, benefits and drawbacks, to the resident or their legal representative.

3

Document the choice

Informed consent is signed and filed. The resident decides; your team processes the form.

4

Plan the way back

We connect residents with insurance specialists for re-enrollment before discharge.

Who we approach, and who we don't

Three checks, every resident

Check 1

Still progressing

Rehab and nursing confirm real gains ahead.

Check 2

Able to decide

Cognition confirmed, or we speak with the legal representative.

Check 3

A safe way back

Plan screened, and re-enrollment planned before discharge.

If any check fails, we don't approach.

What it means for your facility

Better outcomes, built on resident care

Fewer rehospitalizations

Fewer 30-day returns caused by a premature coverage cut.

Stronger hospital relationships

Referral partners see residents leaving stable, not coming back.

Safe, lasting discharges

Residents reach their best level of function before they go home.

Service 2 · Clinical Consulting

Fewer avoidable hospitalizations. Stronger care in your building.

We partner with experienced medical directors, nurse practitioners and clinical nurses to evaluate your hospital transfers and clinical care, then work alongside your staff to safely manage more residents in-house and lower your return-to-hospital (RTH) rate.

Clinical judgment always comes first.

We never discourage a needed transfer. When a resident needs the hospital, they go. Our work focuses only on hospitalizations that, on review, could have been safely prevented or managed in the building, and every treatment decision stays with the resident's physician and care team.
